What should I bring to my first visit?
Please bring a photo ID, your insurance card, a list of current medications, and any records you would like us to review. Arriving a few minutes early helps us get you settled.
How early should I arrive?
Arriving about 15 minutes early gives you time to complete any remaining forms. Completing forms ahead of time online can make check-in faster.
Are you accepting new patients?
Yes. We welcome new patients of all ages. Call or use the form to request your first visit and we will help you get scheduled.
How is my health information protected?
We handle your records in line with HIPAA and our privacy practices. We will explain how your information is used and shared, and you can ask our office any questions about your privacy.
Can I transfer records from my previous provider?
